Logan Health's new OB/GYN Clinic in Whitefish, MT, is looking for a Maternal Care Coordinator - Medical Social Services Advocate to join their team!

What You'Ll Be Doing:

This role is key to improving maternal health by ensuring patients have access to the resources and services they need during pregnancy and postpartum. This position supports patients, families, and caregivers in overcoming social barriers within the healthcare system.

  • Connecting patients with behavioral health services, education, and community resources
  • Supporting patients, families, and caregivers through challenges that affect access to care
  • Providing early intervention and guidance that directly improves outcomes for mothers and families

What You'Ll Need To Have:

  • Bachelor’s Degree in Social Work, or related Social Services field required
  • Experience in a medical or clinical setting preferred.
  • Background working with expecting mothers or postpartum patients is a plus!
  • Current CPR per assigned area(s) may be required
  • Strong interpersonal skills with the ability to handle sensitive and confidential situations professionally
  • Proficient in Microsoft Office Suite with the ability to learn new software
  • Organized, detail-oriented, and able to set priorities while working both independently and as part of a team
  • Excellent verbal and written communication skills for engaging with patients and community partners

Other Job Specific Duties:

  • Addresses identified support and social needs of patients.
  • Exhibits attentiveness of risk for readmission and works with the interdisciplinary team across the healthcare continuum to address social determinants of health that may impact the discharge planning process.  Keeps the patient and identified support person(s) engaged in all aspects of planning.
  • Acts as an expert regarding community resources, healthcare needs impacting discharge plan and social barriers.
  • Explores and communicates solutions and recommendations to social barriers impacting healthcare with appropriate stakeholders. Completes thorough and accurate documentation in the patient’s medical record.
  • As applicable to assigned area(s); Performs as an active member of the healthcare team making early referrals and communicating thoroughly with post-acute partners, family members, and patients; Operates from a person-in-environment framework and strength-based perspective; Exhibits strong, professional communication skills and ensures available community connections are made prior to discharge.
  • Exhibits knowledge and skills necessary to provide for the physical, psychological, and cultural needs of the patient.  Advocates for clients' rights to self-determination, confidentiality, and access to supportive services and resources, and appropriate inclusion in informed decision-making that affects health and well-being.
  • Consistently reflects ownership and professionalism towards Care Coordination.  Identifies areas in need of improvement and serves an active role in developing solutions. Exhibits the Organization’s Core Values to both internal and external customers.
  • Responds to, and provides direction in, a changing workload in a positive and professional manner.

Shift:

Day Shift - 8 Hours (United States of America)

Schedule: Full-time, 40 hours per week, Day shift (8-hour shifts)

Logan Health operates 24 hours per day, seven days per week.  Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
