The Emergency Department (ED) Outreach Specialist Lead is the daily go to person for Better Outcomes through Bridges (BOB) program. Works with Leadership for existing Outreach specialist programs, assisting in program development. The ED Outreach Specialist Lead works in conjunction with patients, team members, hospital staff and community partners to develop and implement strategies that improve the health of individuals who are identified with high utilization of emergency department services. Organizes, collects, reviews and reports health and social information through member home visits and phone outreach, while demonstrating multicultural sensitivity and effective communication skills.

Mental Health Counselor Bachelors *1 FTE Day*

Required Qualifications:
- Bachelor’s Degree: Prefer that degree is in a behavioral health or education field.
- American Heart Association (AHA) Basic Life Support for Health Care Provider completion required, or obtained within one month of hire
- Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request
- In Compliance with company policy and state regulations, a Department of Human Services (DHS) criminal background check may be required for this position, based upon the location of the role. Fingerprinting may also be required
- 4 years working with diverse vulnerable populations, community and/or health care organization.
Preferred Qualifications:
- 6 months direct care experience in a residential and/or outreach position serving vulnerable populations.
