Park Nicollet is looking to hire an RN Care Manager to join our Care Management team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
The RN Care Manager provides patient-centered care management services to patients and families within both the Emergency Center and inpatient settings. This position serves as an integral member of the interdisciplinary healthcare team and will be responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, patient, and patient’s external support system (as applicable). Within the context of an interdisciplinary team, the RN Care Manager will participate in progressing patient towards discharge/next level of care by identifying barriers and implementing appropriate solutions/interventions. The RN Care Manager will work with patients and patients’ external support system to secure appropriate and safe transition plans in keeping with shared goals of care. The RN Care Manager will consistently and thoughtfully apply nursing values and continuous quality improvement in daily work.
Schedule:
- Casual
- The standard hours will be either 7:30-4 or 8-4:30.
- For the casual position, the shifts are only as needed.
Required Qualifications:
- Education, Experience or Equivalent Combination:
Licensure/ Registration/ Certification:
Knowledge, Skills, And Abilities:
- Associate degree in nursing with commensurate experience.
- Prior experience in any of the following areas strongly preferred: utilization management, case management, home care setup, discharge planning.
- Current Minnesota Registered Nurse (RN) license
- Knowledge of the values, principles, and methodologies of nursing
- Ability to integrate social and medical data to make psychosocial assessments and implement appropriate treatment plans.
- Ability to establish and maintain effective working relationships with patients, families, colleagues, and community organizations.
- Sensitivity to cross-cultural issues
- Support and educate other disciplines when appropriate.
- Ability to work effectively as part of an interdisciplinary team.
- Exceptional organizational and time management skills.
- Incumbent must be self-directed and able to prioritize on a continual basis.
- Ability to deal with change and ambiguity
Preferred Qualifications:
Education, Experience Or Equivalent Combination:
- Bachelor’s degree in nursing strongly preferred.
- Minimum of one year hospital-based med-surg nursing experience
