Direct Support Professional- Full-Time/Swing Shift

Position Summary

To participate as part of the Interdisciplinary team, through implementing the Individual Specific Plan and providing feedback on the individual’s progress. To be an advocate for the individual and ensure all health and safety needs are met with dignity.

Position Duties and Responsibilities

1. Contribute to the evaluation of the individual’s progress and provide input to the team with regard to the development of appropriate plans to achieve specific ISP outcomes.
2. Complete written and verbal reports/ documentation related to daily progress.
3. Act as a positive role model to individuals served.
4. Provide life skills support and behavior management 
5. Assist with hygiene, grooming, assist with medication delivery, housekeeping, laundry, cooking and any other areas identified within the ISP.
6. Provide transportation (using personal vehicle) the individual(s) served to access the community.
7. Identify social, economical, recreational, and educational services in the community that will meet the individual’s needs.
8. Provide emotional support and feedback to the individual and their family. 
9. Attend all house meetings, complete all required trainings and work within all HeartWell Policies & Procedures.

Requirements

• Minimum – High school diploma or equivalent
• Current and Valid Driver’s License
• Safe insured/ registered vehicle
