Full time Home health/hospice RN- Utah County/Wasatch Back

Rocky Mountain Care is seeking AMAZING RN's to provide hospice care and case management for our patients in the Utah county/Wasatch back, UT area!

This is a 4 on/3off - 3 on/4 off schedule.

Responsibilities Include:

• Admit patients to hospice care services
• Establish and carry out a plan of care
• Communicate effectively to interdisciplinary team members
• Case management
• Observes/ensures compliance with Medicare and State guidelines
• Discharge planning for Home Care patients
• Attends all IDT and mandatory meetings

Position Requirements:

• Must have a Valid UT RN License
• Reliable Transportation is Required
• Must Possess Clinical, Organizational, and Communication Skills
• Hospice Care Experience Preferred
