Job Summary
The Clinical Pharmacist is responsible for a broad range of clinical and operational activities that support high-quality, member-centered care. Key responsibilities include rounding with provider teams, precepting pharmacy students, conducting medication reconciliation, and tracking member adherence. The role also involves providing member education, supervising pharmacy technicians, and ensuring the accurate filling of prescriptions. Additional duties include managing inventory, coordinating medication packaging and delivery, supporting population health initiatives such as HIV and CKD, and conducting staff in-services to promote best practices across the care team.
Duties And Responsibilities
Clinical Collaboration & Patient Care
- Conduct regular rounding with Provider Teams to enhance interdisciplinary coordination.
- Participate in Population Health initiatives, including but not limited to HIV and CKD care and adherence support.
- Initiate new therapies per applicable collaborative practice agreements and clinical appropriateness.
- Review patient adherence and communicate deficiencies to providers.
- Provide patient education on new medications or devices, high-risk therapies, and adherence concerns.
- Support new member onboarding to pharmacy services.
- Ensure all members filling prescriptions have follow-up appointments scheduled.
Medication Therapy Management & Consultation
- Complete MTM, CMR, and TIP opportunities to optimize therapeutic outcomes.
- Serve as the primary consultant for drug-related inquiries, including renal dosing, drug interactions, and initiation guidance.
- Advise providers on medication selection, dosage adjustments, and side effect management.
- Communicate discrepancies between EMR and pharmacy systems.
- Ensure maintenance of accurate patient records, including allergies and medication reconciliation.
Pharmacy Operations & Safety
- Ensure accuracy and safety of dispensed medications and claim submissions.
- Monitor and log pharmacy errors and review monthly for quality assurance.
- Supervise pharmacy technicians in the absence of the Pharmacist in Charge, ensuring accuracy in all aspects of the fill process.
- Coordinate and supervise prescription deliveries and mailings.
- Oversee inventory control, ensuring availability of necessary medications and removal of expired stock.
- Assist in the annual inventory process.
Medication Coordination & Adherence Tracking
- Track filling, procurement, and scheduling of long-acting injectable medications.
- Coordinate pill tray and blister pack refills, including cycle-based medication reconciliation.
Technology & Systems Proficiency
- Demonstrate proficiency in pharmacy management systems, EMR, and practice management platforms.
- Utilize EMR for reconciliation and adherence monitoring; update medication lists and notify care teams of new prescriptions.
- Demonstrate a good working knowledge of insurance claims payments and resolution.
Education & Training
- Act as primary preceptor for pharmacy students, including onboarding, scheduling, coaching, and assessment.
- Provide training to clinical staff on medication updates, injection techniques, medication handling, and immunization schedules.
- Develop reference materials such as drug monographs, administration guides, and therapeutic updates.
- Demonstrate proficiency in multiple disease states, ex: HIV, CKD, Hepatitis C, CHF, Diabetes, COPD, Asthma, and Heart Disease.
Communication & Customer Service
- Provide excellent customer service to all pharmacy customers.
- Answer and return pharmacy-related calls and messages promptly.
Minimum Qualifications
- Pharm D or Registered Pharmacist.
- 2-3 years’ experience in a similar role.
- Personal and professional passion to help improve healthcare delivery.
- Demonstrated respect for and experience with culturally diverse communities.
- Ability to build and maintain relationships with a diverse population.
- Team and customer service oriented.
Working Conditions
This job operates in a professional pharmacy environment. This role routinely uses general office equipment. Often wear a special uniform, such as a lab coat or surgical scrubs, protective or safety attire, such as gloves and masks. Exposed to diseases nd infections of patients daily. May share workspace with other technicians and staff.
Physical Requirements
- Ability to communicate clearly and exchange accurate information constantly.
- Ability to remain stationary for long periods of time.
- Constantly operates computer, keyboard, copy and fax machine, phone, and other general office equipment.
- Ability to occasionally move objects up to 20 lbs.
