To perform a variety of routine and specialized patient care activities, as assigned, under the direction and supervision of the Registered Nurse (RN). Care will be delivered that incorporates Family Centered Care in a cost-effective manner and is developmental and age-appropriate for the patient population.

 

To perform a variety of routine and specialized patient care activities, as assigned, under the direction and supervision of the Registered Nurse (RN). Care will be delivered that incorporates Family Centered Care in a cost-effective manner and is developmental and age-appropriate for the patient population. ESSENTIAL DUTIES AND RESPONSIBILITIES Responsibility A: Assists with the collection and documentation of patient/family data. • Obtains and documents vital signs (without the use of continuous physiologic monitors) and notifies professional of abnormal findings as evidenced by supervisory observation and review of medical records. • Obtains and documents physiologic values using continuous physiologic monitors; responds appropriately to alarms and notifies professional of abnormal values as evidenced by supervisory observation and review of medical records. • Obtains and documents body measurements and intake and output and notifies the RN of abnormal finding as evidenced by supervisory observation and review of medical records. Responsibility B: Provides high quality, cost effective patient care in collaboration with the RN and in conjunction with the patient/family. • Demonstrates ability to collaborate with multiple RNs in a professional manner to prioritize workload based on patient/family needs as evidenced by supervisory observation and verbal/written feedback. • Provides basic hygienic care as evidenced by supervisory observation, verbal/written feedback. • Provides general comfort measures and performs other miscellaneous ADL’s as assigned as evidenced by supervisory observation, verbal/written feedback. • Ensures effective time/resource management by performing delegated tasks in a timely manner and appropriate utilization of non-busy time, as evidenced by supervisory observation, written/verbal feedback. • Collects and labels specimens for lab analysis and performs bedside testing, including quality control, as evidenced by supervisory observation, written/verbal feedback. • Performs unit specific tasks as outlined in the PCA Orientation Pathway list as evidenced by supervisory observation, written/verbal feedback. • Prepares patient room for patient admission with age appropriate supplies and equipment. • Performs clerical duties, as applicable. • Transports medications as requested, excluding controlled substances, following established medication security guidelines Responsibility C: Assists in identifying patient/family response to care provided. • Provides input to the RN for the development and ongoing modification of the plan of care, as evidenced by supervisory observation, verbal/written feedback, and review of the plan of care. • Assists in identifying patient response to care provided and reports patient condition to the RN to assure achievement of desired outcomes as evidenced by record review and verbal/written feedback. Responsibility D: Maintains accountability for development of self and participates in development of others. • Demonstrates willingness and ability to acquire new knowledge, demonstrate new skills needed to improve individual, team, and organizational performance as evidenced by supervisory observation and verbal/written feedback. • Acts as a resource to new employees and other members of the team to ensure the delivery of quality patient care as evidenced by supervisory observation and verbal/written feedback. • Attends mandatory training and development activities and meets institutional and departmental health and safety requirements by established deadlines as evidenced by supervisory observation and verbal/written feedback. • Remains current in unit/hospital information as evidenced by supervisory observation and verbal/written feedback. Responsibility E: Demonstrates commitment to team, flexibility, professional interpersonal, communication, and customer service skills. • Demonstrates positive and effective interpersonal skills when dealing with patients/families, visitors, peers and other health care team members as evidenced by supervisory observation and verbal/written feedback. • Demonstrates the ability to problem solve, identify appropriate resources, adapt to ongoing changes and handle conflict management with a professional and supportive attitude as evidenced by supervisory observation and verbal/written feedback. • Maintains confidentiality concerning patient/family issues as evidenced by supervisory observation and verbal/written feedback. • Ensures and participates in ongoing communication with other health care team members in order to enhance the patient’s plan of care, advocate for the patient/family and facilitate unit operations as evidenced by supervisory observation and verbal/written feedback. • Performs basic clerical duties (computer, telephone, intercom system), as directed, as measured by supervisory observation, verbal/written feedback. • Prints and labels EKG monitor strips with patient’s name, medical record number, date, and time, as applicable, as evidenced by medical record review and supervisory observation. Responsibility F: Participates in achieving hospital/unit goals. • Demonstrates an understanding of on-going hospital/unit goals, structures and priorities as evidenced by supervisory observation and verbal/written feedback. • Participates in hospital/unit performance improvement initiatives as evidenced by supervisory observation and verbal/written feedback.
