Responsibilities:

• Provide Direct Nursing Care Including Medication Administration And Treatments

• Monitor And Document Patient Health Status; Report Changes Promptly

• Assist With Daily Living Activities While Preserving Client Dignity

• Maintain A Safe And Supportive Home Environment

• Collaborate With Families And Interdisciplinary Care Teams To Implement Personalized Care Plans

Qualifications:

• Active Rn License In New York State

• Minimum 1 Year Of Experience In Home Care Or School Nursing

• Current Cpr/Bls Certification (Aha Or Red Cross)

• Physical Exam Completed Within The Past Year

• Quantiferon Test Within The Past Year

• Mmr Documentation Or Titers
