What You’Ll Do:

• Deliver Direct Patient Care And Treatments Based On Individualized Care Plans

• Administer Medications And Tailored Interventions

• Promote Wellness Through Education, Counseling, And Preventive Care

• Collaborate With Multidisciplinary Teams To Coordinate Care

• Maintain Accurate And Timely Nursing Documentation

• Assist With Mobility And Physical Support (Must Be Able To Lift Up To 50 Lbs)

• Stay Active And Engaged During Extended Periods Of Standing Or Movement

• Take On Additional Duties As Needed To Support Patient Well-Being

What You’Ll Need:

• Active Lpn Or Rn License Issued By New York State

• Minimum 1 Year Of Experience In A Home Care Setting

• Current Cpr/Bls Certification

• Strong Communication And Organizational Skills

• Empathy, Reliability, And A Commitment To High-Quality Care
