Description
Under the supervision of the Medical Operations Supervisor, supports the Medical Services Director and Advanced Practitioners in the admissions process across programs, assists in regulatory processes and ensures quality of care to Janus patients across programs. Assumes additional responsibility as needed in consultation with the Medical Services Director.
GENERAL FUNCTIONS
- Checks and documents client vital signs at admission and when indicated.
- Orders withdrawal management and other medications from the pharmacy as directed by Medical Director and Advanced Practitioners.
- Depending on patient acuity, periodically observes clients for signs of mental or physical decompensation that may require escalation to emergency services.
- Assists clients with health questionnaire and conveys detailed information regarding medical history to Advanced Practitioner.
- Records basic information about current conditions of clients.
- Reports all Medical Incidents to Program Manager, Medical Services Director, or designee.
- Respects and protects the Human Rights of patients.
- Follows up on clients medical needs as identified by the Advanced Practitioner or Medical Services Director.
- Scans medical documents into EHR by close of business daily (ie. health screening questionnaire, physical, DHCS clearance forms, etc.)
- Receives and documents prescription medication from 3rd party care providers by:
- Recording prescribed medication on medication log or IMS health questionnaire (page 7)
- Completing medication log sheets for the Medication Administration Record (MAR).
- Welcomes new clients into the facility in a professional, inviting and courteous manner
- Collects medical records from different providers for all clients reporting a history of hospitalization within the last year
- Screens and tests for TB, providing referrals as appropriate.
- Conducts health education classes for clients and patients.
- Provides medications to clients in residential program as directed.
- Assists and coordinates medical care to clients in across all Janus programs and conduct educational sessions if assigned.
- Coordinates care and adheres to state requirements for pregnant clients in methadone maintenance program if assigned.
- Attends all required training and administrative meetings.
- Maintain all required certifications professional licenses/certificates.??
- Driving is required to attend required training courses, meetings or to fulfill other responsibilities defined in this role.
RELATED FUNCTIONS
- Demonstrates an understanding of physiological effects commonly misused substances and the pharmacology of medications used to manage SUD (substance use disorder).
- Works closely and communicates clearly across Janus programs and referral resources to provide quality client care.
- Consistently documents information in computer, electronic health records, and other record keeping systems to ensure all forms are complete and up to date according to agency policy.
- Maintains compliance with all Title 9, Title 22, CARF, DEA and CSAT regulations.
- Keep confidentiality of client records and privileged information.
Requirements
- Makes or receives calls related to client care
- Medical Assistant or LVN with at least one year of experience preferred.
- Proficient computer skills and ability to learn and implement Electronic Medical Software (AVATAR) to perform essential job functions which will include thoroughly documenting and entering program notes in a timely manner.
- Demonstrate a strong ability to communicate and interact with a broad range of individuals I a professional manner, good conflict resolution and communication skills, ability to complete tasks in a timely manner, and ability to work as part of a treatment team.
- Ability to pass a background test and a TB test.
- Personal and intellectual ability to collaborate and organize effectively, understand and work with complex systems.
- Cognitive and intellectual capacity to comprehend the organization & facility policies and procedures.
- Physical ability to safely perform job functions with or without the aid of mechanical devices.?
- Manual dexterity sufficient to operate a computer or manually complete required documentation in a legible and timely manner.?
- Ability to effectively perform job responsibilities in a high stress environment.?
- Frequently moves equipment weighing up to 50 pounds for various needs.
- Must be able to remain in a stationary position 50% of the time.
- Needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
