A Client Care Navigator (CCN) is a very important role within the Isaiah House organization. Navigators represent the treatment programs offered by Isaiah House to front-line referral agencies throughout the state of Kentucky, specifically Fayette, Jessamine, Clark, Madison, Garrard and Scott counties.

JOB SPECIFIC FUNCTIONS:

• Build strong relationships with Centers of Influences (COI) in the CCN assigned Region
• Educate COIs on the history, programs and outcomes provided by Isaiah House
• Field Calls from potential clients who are exploring treatment options
• Manage travel schedule to counties specifically assigned
• Maintain database on all COI activity, Client referrals, Clients admitted in ASANA
• Communicate all new client referrals through CCN supervisor via MS Teams

Requirements

QUALIFICATIONS:

• High school diploma or GED preferred
• Knowledge of addiction and substance abuse treatment
• Connections with community services, organizations, and other public groups is essential
•  Must have a minimum of 12 months continuous sobriety and demonstrate ongoing recovery.
