It’s a really exciting time to join us as we continue to deliver the Heathier You NHS Diabetes Prevention Programme (NDPP) throughout North-West London and we’re looking for passionate health coaches (or Diabetes Prevention Practitioners) who are fluent in Gujarati or Bengali to join us as part of our bank team, to primarily support our South Asian Community!

The purpose of a health coach is to improve lives, through empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes. This is achieved through successfully delivering our quality programmes and providing evidence-based information, supporting individuals to improve their health, lose weight and ultimately lower their risk of developing diabetes.

A typical day as a Health Coach might include:

  • Delivering evidence-based group sessions to diverse client groups that focus on diet, nutrition, physical activity and behaviour change. Both running your own groups and also covering sessions for other health coaches as required.
  • Facilitating group intervention sessions to support your clients in achieving their goals and cultivating a safe and positive environment, with additional administrative work as necessary.
  • Building relationships within the South Asian Community to promote healthy lifestyle changes, demonstrating cultural sensitivity in client interactions whilst creating a welcoming environment for all our Service Users.
  • Supporting the delivery of our Tailored Remote service for South Asian speakers (delivered in Urdu, Punjabi and Bengali, with growth for Gujarati and Hindi speakers)
  • Promoting the Healthier You NHS Diabetes Prevention Programme internally and externally, and improving referral pathways between other services and partner organisations
  • To carry out triage work as required, particularly in desired languages and possible translation roles, whilst ensuring Thrive Tribe benefits from linguistic versatility and cultural insights.
  • Maintaining professionalism, attention to detail, punctuality and good communication skills.
  • Engaging in ongoing training relevant to your role, such as Behaviour Change Level 1 and 2
  • Working towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures
  • Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
  • Ensuring due consideration and adherence to our internal Thrive Tribe policies, including Information Governance, Safeguarding and Complaints procedures
  • Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!

As a key aspect of your role, you will be responsible for transporting equipment, such as scales to and from various community venues.

We’re looking for someone who can combine health improvement knowledge with first class communication and motivational skills. If you have experience of delivering group-based interventions, and a real understanding of the impact personal characteristics and circumstances can have on health behaviours then we want to hear from you. Bonus points if you have a qualification in a health promotion related subject, such as nutrition, exercise, or health coaching!

Although not essential, so that we can best support the needs of our clients in North London we are particularly keen to hear from applicants who are fluent in Polish, Portuguese, Romanian, Urdu, Spanish, and/or British Sign Language for these roles.

We hire based on attitude over traditional skills and experience so if you don’t have all of the necessary experience but feel you are right for the job we would love to hear from you!

Due to the nature of this role, we’re looking for someone who is seeking a varied and flexible working pattern including evenings and weekend work.

Contract: Zero hours contract

Our Diabetes Prevention Programme consists of 13 sessions over a 9 month period, ordinarily we expect that you will be delivering multiple sessions per week.

Salary: £13 - £20 per hour

Location: This is a hybrid-remote role. There is no office base so the role will require some home working, with regular travel across North West London.
