Description

WeCare Centers at South Hills Care Center is seeking an experienced LPNAC to join our team on a full-time basis. In this role, you’ll support our nursing facility by completing MDS assessments and ensuring accurate documentation for quality resident care and proper reimbursement.

Key Responsibilities

What You’ll Do: 

• Complete MDS assessments (MDS experience required) 
• Prepare and transmit reports to Medicare & Medicaid 
• Ensure accuracy of medical records and coding 
• Conduct care area assessments and care planning 

Skills, Knowledge And Expertise

• Licensed Practical Nurse (LPN) with MDS experience 
• Strong attention to detail and accuracy 
• Ability to work independently and remotely
