We're looking for a driven and detail-oriented professional to join our team as a Family Medicine Nurse Practitioner (APRN-CNP). In this role, you'll play a vital part in providing expert medical care to patients of all ages with a range of potential illnesses, injuries, or disabilities, helping us move forward with purpose and precision. Essential Job Duties:

  • Provide comprehensive primary care services to patients of all ages, including diagnosing and treating acute and chronic illnesses, managing chronic conditions, and promoting preventive health.
  • Conduct thorough medical assessments, including taking detailed medical histories, performing physician examinations, ordering, and interpreting laboratory tests and diagnostic imaging, and making accurate diagnoses.
  • Develop and implement evidence-based treatment plans for patients, including prescribing medications, providing counseling on lifestyle modifications, and coordinating referrals to specialists when needed.
  • Serve as an advocate for patients, promoting their rights and assisting them in navigating the healthcare system, including obtaining insurance coverage and accessing community resources.
  • Collaborate with colleagues and participate in interdisciplinary meetings, case conferences, and other team-based activities to ensure coordinated patient-centered care.
  • Maintenance of all required licensure, certification, registration, training, medical knowledge (including but not limited to new or changing treatments and standards of care), and or expertise to meet the needs of patients of Third Street as determined by the organization.

Requirements

Qualifications:

  • Current Ohio APRN license
  • Board Certified or provide letter indicating qualification to sit for certification with expectation of obtaining certification within one year.
  • Valid Driver’s License

Environmental & Physical Requirements:

  • Physical Factors:              Occasionally move or transport items weighing up to 20 lbs.              Carrying: up to 20 pounds occasionally              Frequent movement required in this position while carrying equipment.              Movement: 5-10 minutes at a time throughout the day              Stationary: 15-60 minutes at a time              Frequently requires the ability to observe details at a close range.              Driving: may occasionally do limited driving              Vision: color distinction necessary              Hearing: no greater loss than 40dB average for 500, 1000, 2000 Hz
  • Mobility: must be able to move independently and safely throughout the workplace, including offices, counseling rooms, and common areas.
  • Manual dexterity: should have sufficient hand-eye coordination and fine motor skills to perform tasks such as writing, typing, operating computer systems, and handling paperwork.
  • Visual and auditory abilities: must have adequate vision and hearing abilities to effectively communicate with clients, read written materials, and interpret non-verbal cues during counseling sessions.
  • Speech and communication: should possess clear and effective verbal communication skills to facilitate open dialogue with clients, colleagues, and other healthcare professionals.
  • Sitting or standing: should be able to sustain a seated or standing position for extended periods while meeting with patients/clients/external visitors, attending meetings, or performing administrative tasks.
  • Emotional resilience: must have the emotional stability and resilience to work with clients/patients/coworkers facing challenging circumstances, maintaining composure and professionalism during difficult discussions.
  • Sensory sensitivity: should be sensitive to the needs of clients/patients/coworkers and exhibit appropriate responses to their physical and emotional cues, ensuring a supportive and non-judgmental environment
