Wraparound Facilitator - Position Overview

  • Wraparound is a care coordination process that builds on the collective action of a committed group of family, friends, community, professional, and cross-system supports mobilizing resources and talents from a variety of sources to address the needs of children/youth with complex mental health challenges and their families.
  • The Canopy Wraparound Facilitator is responsible for engaging the family and developing a strength-based relationship to identify, create and build a supportive Wraparound Team for the purpose of developing a holistic single plan of care in accordance with high fidelity and quality wraparound practice.

Job Responsibilities:

  • Engage the family to begin the process of high-fidelity Wraparound and educate family, child/youth, and new team members about the Wraparound process and components of wraparound.
  • Gather information for the development of the family story and assist family with developing their family vision to present at the first 30 day Wraparound Team Meeting.
  • Complete the initial Crisis Management Plan with the family within 24 hours of admission to Wraparound.
  • Engage families in order to identify natural/formal/informal supports for the family and child/youth and have them present at the initial WTM.
  • Responsible for maintaining team cohesiveness and providing an environment that is conducive for all team members to have supportive and positive communication between the team and families.
  • Facilitate a Wraparound Team Meeting every 30 days at a minimum, more frequently in the initial stages of development if needed, and as often as needed to provide quality services.

Required Qualifications:

Family Partnership & Tiered Caseload

  • Maintain a modified family caseload based on supervisory duties:
  • Deliver high-fidelity Wraparound services, ensuring alignment with model principles such as family voice and choice, team-based planning, and strengths-based care.
  • Modified caseload depends on the number of supervisees and families they are partnered with. 
  •  1 supervisee – partner with up to 8 families
  •  2 supervisees – partner with up to 5 families
  •  3 supervisees (maximum) – partner with up to 3 families

In line with high-fidelity Wraparound principles, the Wraparound Lead will: 

  • Engage families to initiate the Wraparound process, educating them and all team members on Wraparound principles and expectations. 
  • Gather family and system information to develop the Family Story and Family Vision for presentation at the first Wraparound Team Meeting (WTM). 
  • Complete the Crisis Management Plan within 24 hours of admission. 
  • Identify and engage natural, informal, and formal supports to participate in WTMs. 
  • Facilitate WTMs at least every 30 days (or more often if needed) to ensure progress and engagement. 
  • Prepare and distribute all WTM materials (agenda, handouts, plans) and ensure team participation or feedback. 
  • Provide follow-up between WTMs with all team members to monitor tasks, identify/remove barriers, and revise the plan as needed. 
  • Facilitate development of the Wraparound Plan, ensuring it reflects family voice, strengths, crisis planning, and progress toward desired outcomes. 
  • Ensure the family receives, reviews, and signs off on the Wraparound Plan after each revision. · Offer 24/7 support through on-call services (may require evening/weekend work). 
  • Link families to community resources and referrals as needed to meet underlying needs. 
  • Help families access transportation when necessary in a safe, responsible manner. 
  • Guide the team to ensure supports and services remain aligned with evolving family needs. 
  • Engage in regular coaching, training, and supervision, including state and agency Wraparound-specific sessions. 
  • Participate in CREST evaluations, implement supervisor feedback, and complete documentation and reports as required. 
  • Support marketing/outreach efforts and perform quality assurance activities as assigned. Staff Support & Team Leadership 

Staff Support & Team Leadership:

  • Provide reflective supervision and coaching to up to 3 assigned Wraparound Facilitators. 
  • Conduct routine check-ins, support staff morale, and troubleshoot barriers with Facilitators. 
  • Onboard and mentor new Facilitators.
  • Ensure fidelity to Wraparound values and Medicaid/DMH standards through coaching and consultation.
  • Attend team supervision, facilitate peer learning, and promote positive, collaborative culture. 
  • Participate in chart reviews and fidelity audits (using Supervisor Checklist, COMET) and help address performance gaps. 
  • Provide crisis support backup for Facilitators when they are unavailable.
  • Monitor team caseloads for admissions, discharges, and all KPIs. 
  • Monitor all regional incidents, follow report protocol, and ensure all stakeholders are notified. Administrative & Cross-Agency Collaboration

Administrative & Cross-Agency Collaboration

  • Collaborate with Wraparound Supervisors and Directors to support operational goals, regional KPIs, solution KPIs, and organizational KPIs.
  • Help track team KPIs and assist with monthly reporting and data validation.
  • Promote team-wide communication and coordination with CPS, MYPAC, Admissions, and other partners. 
  • Support smooth transitions and warm hand-offs across service levels. 
  • Collaborate with admissions team for all referral, outreach, and utilization management related duties. 
  • Other duties as assigned.
