The MMCP Foster Care Liaison must have experience, expertise, and knowledge of the child welfare system, foster care healthcare requirements, and the unique complex needs (including needs resultant of trauma) of this population. It is required to have knowledge and understanding of the behavioral health landscape within New York State. This includes familiarity with various levels of care, different types of services, and the relevant state systems for both children and adults. While a physical location in New York State might not be explicitly required, the staff member needs to demonstrate competency in navigating and understanding the state's specific behavioral health system. LMSW, LCSW, LMHC and LMFT licensure highly preferred.

Position Purpose: The Senior Health Liaison will work closely with the Senior Director of Foster Care Operations, Foster Care Product Implementation Manager and Director of Care Management to increase engagement with partners and providers. The Senior Health Liaison will oversee Pilot Projects that reside in the Foster Care Operations side of the plan to increase our reunified members who remain on our AHCC plan. The Senior Health Liaison will additionally spearhead messaging for our alumni population outreach. The Senior Health Liaison will be responsible for health equity initiatives for our members. They will retain all the responsibilities of the Health Liaison as well as training and onboarding new Health Liaison staff. Facilitating strategy between Coordinated Care and DCYF.

  • Serve as the primary contact for the health Plan Network (SHPN) with external stakeholders, community organizations and judicial systems that traditionally interface with Child Protective Services (CPS).
  • Collaborate and consult with DFPS Well Being Specialists to troubleshoot complex cases, analyze process effectiveness, and recommend solutions.
  • Serve as the subject matter expert for the foster care medical and behavioral health care contract and services.
  • Delegated to oversee Health Liaison education IFC Pilot Projects
  • Serve as Health Liaison Health Equity Initiatives SME

Education/Experience: Bachelor’s degree in Social Work and 5+ years of experience working within a social service agency and/or advocacy environment. Direct experience in child welfare and Indian Child Welfare. Highly organized and ability to work in everchanging situations. Ability to be flexible.

Pay Range: $68,700.00 - $123,700.00 per year
