Description

 

Performs medication order processing responsibilities within established policies, procedures and guidelines and provides drug information while applying a thorough understanding and knowledge of drug and medical therapy .

 

Qualifications

 

Minimum Education

Completion of a five year pharmacy program in an accredited college of pharmacy. (Required)

  • Medication Processing
  • Interpret medication orders, profile orders and oversee the packaging, labeling and distribution of medications.

Drug Intervention and Education

  • Perform drug therapy monitoring and intervene as necessary on drug dosing, drug administration, drug interactions, side-effects and monitoring issues; complete therapeutic drug monitoring consults and adverse drug reaction reports as assigned.
  • Provide accurate and timely drug information to physicians, nurses and other health professionals.
  • Interpret research medication orders, ensure accurate preparation of investigational drugs and maintain complete and accurate records.
  • Must maintain proficiencies in medication handling (including hazardous medications), compounding sterile/non-sterile preparation(s) and completing quality assurance checks per standard operating procedures and policies in compliance with USP <795>, <797> and <800> guidelines.

Supervision/Training

  • Oversee the work of Pharmacy Technicians by directing and assisting as necessary to ensure rapid medication order turn-around time.
  • Assist with the education, training and general supervision of Pharmacy Residents, Students and Technicians.

Safety

  • Speak up when team members appear to exhibit unsafe behavior or performance
  • Continuously validate and verify information needed for decision making or documentation
  • Stop in the face of uncertainty and takes time to resolve the situation
