POSITION SUMMARY: The Director of Care Management is responsible for the implementation, management and education of care coordination activities for all patients, clients, and communities served by PMS clinical programs, those served in partnership with the New Mexico Care Partners Clinically Integrated Network, and other contractual partners. Addresses the triple aims of improving patient experience, improving the health outcomes of populations, and reducing the per capita cost of health care.

Requirements

Education And/Or Experience:

  • Master’s or Bachelor’s degree; Master of Business Administration highly desirable
  • Minimum six years’ experience in managed care related field, including healthcare plans and case management
  • Minimum three years management/supervisory experience
  • Knowledge of managed care rules and payment structures within the state of NM
  • Ability to interpret contract, benefits, and claim payment guidelines
  • Understanding and implementation of value-based care strategies.

Certificates, Licenses, Registrations:

  • Project Management professional certification strongly preferred.
  • Care Coordination and Transition Management (CCTM) certification strongly preferred.

Screening Requirements:

  • This position requires successful completion of an initial post-offer of employment Criminal Records Check.
