Medicaid Eligibility Specialist/Care Manager Extender

Pay Scale: $16-$17.50/hr.

Summary:

Under direct and indirect supervision, provides outreach and assistance to clients to ensure Medicaid eligibility.

Essential Duties and Responsibilities

• Performs outreach to identified Tailored Care Management (TCM) clients who are in need of Medicaid re-certification. Provides direct assistance to client to complete needed paperwork to ensure Medicaid re-certification is complete and benefits are maintained. Works closely with other TCM staff to ensure client’s Medicaid benefits are maintained. Also assists clients with no insurance or Standard Plan Medicaid transition to MCO/Tailored Plan Medicaid when appropriate.
• Keys in important client data to schedule appointments. Successfully link clients to center care management staff to ensure appropriate follow up and participation in services.
• May file, release and copy protected healthcare information. Answers phone calls, transfers to the appropriate parties, is courteous to the public.
• All other duties as assigned by supervisor

Responsibilities of the TCM Medicaid Eligibility Specialist include, but are not limited to, the following:

• Performs outreach to identified Tailored Care Management (TCM) clients who are in need of Medicaid re-certification. Provides direct assistance to client to complete needed paperwork to ensure Medicaid re-certification is complete and benefits are maintained.
• Works closely with other TCM staff and if needed other Daymark departments to ensure Medicaid benefits are maintained.
• Utilize NCTracks system to verify coverage.
• Assist clients with no insurance or Standard Plan Medicaid transition to MCO/Tailored Plan Medicaid when appropriate

Special Attributes:  Must possess a good attitude, work well with others, have a strong commitment to patient care, understand office flow, understand budgeting and have knowledge of Mental Health, Social Work or Health Care Fields.

Education And/Or Experience:

TCM-Extender qualifications: high school diploma or associate degree with experience working with adults or children with mental health or substance use disorder needs or lived experience.
