Responsibilities

Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. Purpose-built to serve patients transitioning from hospital to home, Burnett includes specialized units — Memory Care, Long-Term Care and Transitional Care — that focus on quality, comfort and continuity of care. This environment offers nurses the opportunity to provide patient-centered care across diverse needs in a collaborative, post-acute setting. 

  • Support the Minimum Data Set (MDS) assessment process by gathering, documenting and entering resident assessment data to ensure accuracy and completeness. 
  • Complete assigned MDS sections with precision and timeliness, ensuring all documentation meets state and federal standards while supporting quality resident outcomes. 
  • Partner with the MDS Coordinator, interdisciplinary team and nursing leadership to promote a smooth, compliant assessment process and resolve any documentation discrepancies. 
  • Maintain up-to-date knowledge of MDS guidelines, integrating changes into daily practice to ensure ongoing compliance and optimal resident care planning.

Qualifications

Education & Experience

  • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required 
  • Completion of an accredited Vocational Nursing School program OR become licensed by equivalency required 
  • 2 years of Long Term Care experience required 

Licenses & Certifications

  • LVN - Current State of California Licensed Vocational Nurse License required 
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
