SearchPlus HR is on the lookout for a dedicated Clinical Pharmacist to strengthen our healthcare team. In this important role, you will utilize your clinical expertise to enhance patient care through optimal medication management.

As a Clinical Pharmacist, you will assess medication orders, provide clinical consultations, and educate patients and healthcare staff about various medications. You will be an essential member of the interdisciplinary team, working collaboratively to ensure the safe and effective use of pharmaceuticals and improve patient outcomes. Your contribution will directly impact the quality of care we deliver to our community.

Key Responsibilities

Review and evaluate medication orders for safety, appropriateness, and therapeutic efficacy.
Provide patient counseling on medication use, risks, benefits, and adherence strategies.
Collaborate with healthcare professionals to design and implement individualized medication plans.
Conduct medication therapy management and monitor patient responses to therapies.
Participate in clinical rounds and contribute to the healthcare team's decision-making process.
Document clinical activities and ensure compliance with pharmacy regulations and standards.
Engage in continuing education to stay current with advances in pharmacotherapy.

Requirements

Doctor of Pharmacy (PharmD) degree from an accredited institution.
Active pharmacy license in the state of practice.
Prior experience in clinical pharmacy is preferred.
Strong understanding of pharmacotherapy and medication management.
Excellent communication, presentation, and interpersonal skills.
Detail-oriented with strong analytical and critical thinking abilities.
Ability to work effectively in a dynamic healthcare environment.
