This role offers the flexibility to work as needed while making a meaningful impact on patients and families in a healthcare environment.Apply today — contribute to patient-centered care on your schedule!Position Overview:The Social Worker MSW PRN effectively communicates and collaborates with all members of the healthcare team to ensure patients receive safe, timely services, and families are provided with necessary resources and education. This role involves assessing psychosocial risks, providing mental health evaluations, and supporting complex discharge planning. The social worker also serves as a key point of contact for cases involving abuse, neglect, guardianship, foster care, and community resources, acting as a liaison with protective services and other agencies.Key Responsibilities:
  • Collaborate with healthcare team members to facilitate safe, effective, and timely patient care.
  • Interpret and communicate complex patient and family needs, modeling caring practices.
  • Assess psychosocial risk factors through evaluation of prior functioning, support systems, and coping abilities.
  • Counsel patients and families on emotional, social, and financial impacts of illness or disability.
  • Access and mobilize family and community resources to meet patient needs.
  • Conduct mental health assessments upon physician referral and deliver therapeutic interventions.
  • Support complex discharge planning, including evaluations related to abuse, neglect, guardianship, foster care, and adoption.
  • Serve as the point person for cases involving child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
  • Liaise with protective agencies, guardianship, foster care, and mental health providers.
  • Utilize crisis intervention and de-escalation techniques as needed.
  • Develop and maintain relationships with community organizations, law enforcement, schools, and other stakeholders.
Qualifications & Requirements:
  • Education: Master’s Degree in Social Work (MSW) required.
  • Licensure: Permanent LSW licensure required; LCSW preferred.
  • Experience: 0-3 years in healthcare, mental health, or community-based settings preferred.
  • Certifications: De-escalation certification preferred.
  • Skills & Knowledge:
    • Knowledge of the impact of disease processes and human behavior for psychosocial assessments and crisis intervention.
    • Familiarity with state and county legal mandates regarding child and adult protective services.
    • Principles of growth and development across the lifespan.
    • Strong crisis de-escalation and conflict resolution skills.
    • Ability to develop strong working relationships with community agencies and healthcare teams.
    • Proficiency in Microsoft Excel and Word.
