MDS Nurse (LPN or RN) – Cincinnati, OH

We are seeking a detail-oriented and motivated MDS Nurse (RN or LPN) to join our skilled nursing team in Cincinnati. This role plays a key part in ensuring accurate and timely completion of resident assessments and supporting our commitment to quality care.

Responsibilities:

• Coordinate and oversee the Resident Assessment Instrument (RAI) process. 
• Accurately complete and submit MDS assessments within required timelines. 
• Work collaboratively with the interdisciplinary team to ensure compliance with state and federal regulations. 
• Monitor care plans and provide input for ongoing improvement in resident outcomes. 
• Maintain knowledge of current reimbursement and regulatory guidelines. 

Qualifications:

• Licensed Practical Nurse (LPN) or Registered Nurse (RN) with active Ohio license. 
• Previous MDS experience preferred, but we are willing to train the right candidate with strong clinical and organizational skills. 
• Knowledge of state/federal guidelines and reimbursement practices a plus. 
• Strong attention to detail, communication, and teamwork skills.
