- Job Title: MDS Registered Nurse (RN)
- Location: Booker T. Washington Nursing Home, Louisiana
Job Summary:
The MDS RN will be responsible for conducting comprehensive assessments of residents to ensure the highest quality of care and compliance with regulatory requirements. This role involves collaboration with a multidisciplinary team to develop individualized care plans, monitor resident progress, and ensure that documentation meets the standards set forth by CMS and other governing bodies.
- Key Responsibilities:
- - Conduct and coordinate the Minimum Data Set (MDS) assessments for residents, ensuring accuracy and completeness.
- - Develop, review, and update comprehensive care plans based on assessment findings and resident needs.
- - Collaborate with nursing staff, physicians, and other healthcare professionals to implement and evaluate care plans.
- - Monitor residents’ health status and respond to changes in condition, making necessary adjustments to care plans.
- - Ensure compliance with federal, state, and local regulations, as well as facility policies and procedures.
- - Participate in quality improvement initiatives and assist in training staff on MDS-related processes.
- - Maintain accurate and confidential resident records and documentation in compliance with HIPAA regulations.
- - Educate residents and their families about care plans, services, and health maintenance.

Qualifications
- Qualifications:
- - Current Registered Nurse (RN) license in the state of Louisiana.
- - Previous experience working with MDS in a long-term care setting preferred.
- - Strong knowledge of CMS regulations and the long-term care process.
- - Excellent assessment, communication, and interpersonal skills.
- - Ability to work effectively as part of a team and independently.
- - Proficient in electronic health record (EHR) systems and Microsoft Office Suite.
