HIM Birth Certificate Clerk - Per Diem
Shift: Variable 8 hour shifts
*Bilingual - Must be able to speak and write fluent Spanish language.
Job Description:
PATIENT POPULATION:
The patient population served is all patients including geriatric, adult, adolescent, pediatric, and newborn. This also includes services which affect facility staff, physicians, visitors, vendors and the general public.
POSITION SUMMARY:
Under the Direction of the Director of Health Information Management (HIM), the HIM Birth Certificate Clerk interviews parents of newborns to accurately complete a birth certificate, obtaining all necessary signatures and filing certificates in a timely manner.
Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call. Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours. As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act.

EDUCATION/TRAINING/EXPERIENCE:

To perform this job successfully, an individual should be a high school graduate or equivalent. One (1) or more years of previous similar hospital experience, or equivalent education, i.e. Medical Assistant certification, HIT certification. Knowledge of JCAHO and state charting requirements.
Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence if required. Effective oral and written English language communication and interaction skills. Bilingual in Spanish required.
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required.
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Effective time management skills to permit working in a fast-paced, results-oriented environment. Orientation to detail.
To perform this job successfully, an individual should type wpm 40-50 wpm. Computer terminal experience and basic working knowledge of commonly used business software (including but not limited to Microsoft Office, Word, Excel, and e-mail).
LICENSURE/CERTIFICATIONS:
To perform this job successfully, an individual should be a high school graduate or equivalent.
Responsibilities and Essential Functions:
*Indicates Essential Function
1 * Obtains information pertinent to child/parent from the Obstetric and Pediatric records located in the Maternal Child Health Department and from parents. Must process birth certificates within 24 hours of admission.
2 * Prepares a correct and legible birth certificate; reviews for accuracy and completion, with 5% or less error rate returned from county regulatory agency due to incomplete information.
3 * Files certificates with local Registrar within ten (10) days of completion.
4 * Secures all necessary signatures for proper filing with local and state agencies.
5 * Assists parent(s) of children born in the Hospital by completing any forms or statements needed to correct errors on the original birth certificate.
6 * Maintains compliance with state birth certification regulations in filing, compleetion, and submission.
7 * Is familiar with Pediatric and Acute facility and skilled Medical Record requirements.
8 * Be in attendance on site and arrive to work on time.
