Your part in our UnitingCare team
We are currently seeking to appoint a Case Worker to work as a part of The Gardens team. Our Case Workers support families experiencing or at risk of homelessness with housing case management whilst they are in short-term accommodation. This position will support our weekend staff and act as an on-call for shifts during the week.
As part of this role, you will:
Deliver rapid and coordinated assistance to people in critical need of housing and/or support through triage, assessment, case planning and crisis intervention.
Respond and action referrals identified during case management with families.
Adhere to program guidelines regarding the appropriate use of allocated funds.
Undertake administrative duties related to the operation of the program, including maintaining clear, concise and accurate records within the appropriate timeframes.
Identify and inform line management of current or potential risks that may impact on service delivery and the safety of clients and staff.
Positively represent the organisation when engaging with all stakeholders.
Undertake other duties as and when required within the scope and capabilities of the role.
What makes a difference for us?
A tertiary qualification (Diploma minimum) in human services, community services, Youth Services or related field is essential
You must have excellent verbal communication skills that enable you to assess needs accurately and extensively as well as transferable written communication skills to record assessments on the database.
Ability to actively engage with people who have a range of needs and to respect the cultures of all people you support and work alongside.
Ability to work collaboratively as part of a team as well as responsibly within an autonomous setting.
Demonstrated competence with technology, particularly Microsoft Office and tools for data entry.
Working with Children Check (Blue Card)
Current Open Drivers Licence.
As this position will support our weekend staff and act as an on-call for shifts during the week, must have the ability to work weekends.
