Job Description:

Position Overview:
Reporting to the Manager of Ambulatory & Community Mental Health Programs, the Intake Worker will provide intake and navigator services to the Outpatient Mental Health Program. Working with the Central Intake team, the Intake Worker is responsible to:

Responsibilities:
• Receive, review and process internal and external referrals utilizing the Central Intake model
• Complete intake assessments, obtaining collateral information and directing referrals to appropriate treatment pathways
• Collaborate with Intake team to support complex or priority referrals
• Arrange and communicate internal appointments and dispositions with referral source and the patient
• Provide support and collaboration to external referral sources to assist with navigation of referrals deferred to the community. Ensure recommendations are communicated effectively and in a standardized written and verbal manner.
• Facilitate internal referrals to the appropriate treatment pathway, collaborate with treatment teams to ensure excellent communication and seamless transitions
• Data collection, stewardship of wait lists and gaps in service
• Documentation utilizing EPIC

Requirements:
• Completion of a relevant 2-year Community College Degree
• Knowledge of specific symptoms of major mental illnesses
• Minimum 2 years of previous experience working in a mental health setting
• Knowledge of Risk assessments
• Sound knowledge of community resources and partners
• Knowledge of the Mental Health Act, PHIPPA, and protection legislation
• Sound working knowledge of current hospital functions
• Excellent verbal and written communication skills
• Strong interpersonal, organizational and problem-solving skills
• Familiarity with community partners and mental health agencies in Scarborough
• Ability to work well within an inter-professional team and to foster effective working relationships and community partnerships
• Ability to exercise initiative and observe strict confidentiality
• Demonstrated ability to be sensitive to cultural and ethical issues
• Cultural competence and experience collaborating with diverse communities
• Efficient in the use of MS Word, Excel, Internet browser and EPIC
• Demonstrated understanding of and commitment to Scarborough Health Network’s comprehensive safety programs and practices including staff, patient and environmental safety and participate in mandatory training
• Proven good attendance record and ability of maintaining same standard
