Description
As the leader of the care team, the PCP Provider delivers outstanding, comprehensive primary health care to a panel of patients. The Provider offers medical oversight, expertise, and leadership to ensure the highest quality, affordable care for their community.
- Provide patient-centered, comprehensive primary care services consistent with a value-based care model.
- Conduct regular health assessments, diagnose and treat common illnesses, and manage chronic conditions.
- Actively participate in practice quality improvement programs and evaluate practice performance metrics.
- Adhere to privacy, compliance, and quality programs, as well as other regulatory and compliance standards of HealthyU Clinics. Comply with all policies, procedures, protocols, and our Code of Conduct.
- Participate in the implementation of new health technologies, products, services, and programs that establish HealthyU Clinics as a recognized leader in community health and wellness.
- Collaborate with other healthcare professionals to provide coordinated and comprehensive care.
- Provide preventive care and health education to patients.
- Maintain detailed patient records and ensure accurate documentation.
- Perform minor surgical procedures and respond to medical emergencies.
- Stay up-to-date with medical advancements and best practices by attending conferences and seminars.
- Perform other duties as assigned.
Requirements
- Completion of a Medical program in Family Medicine or Internal Medicine.
- Valid, state-issued license for the state in which you will work (license must be active on the first date of work).
- Current BLS (Basic Life Support) certification.
- Current DEA Registration.
- Two professional references (HealthyU reserves the right to make adjustments to this job description based on business needs).
- Minimum of two years of clinical practice experience.
- Experience providing primary (longitudinal) care.
- Knowledge of and experience with managed healthcare systems, including quality assurance, quality improvement, risk management, and population health and disease management programs.
- Ability to work collaboratively with a multidisciplinary team.
- Superior communication and interpersonal skills.
- Self-assured, results-oriented, and committed to continuous professional development.
