This role involves interviewing patients and families to gather social history, assisting with environmental difficulties, and serving as a liaison between medical teams, patients, families, and outside agencies. The Social Worker interprets and helps resolve social issues related to medical conditions and hospitalization, supporting optimal patient outcomes.Key Responsibilities:

• Interview patients and relatives to obtain relevant social history for medical and discharge planning.
• Assist patients with environmental and social challenges that may impede care and recovery.
• Serve as a liaison between healthcare teams, patients, families, and external agencies.
• Interpret and resolve social problems related to medical conditions and hospitalization.
• Collaborate with medical and nursing staff to develop and implement patient care plans.

Qualifications & Requirements:

• Education:
  • Bachelor’s Degree in Social Work (BSW) required.
• Experience:
  • 0-3 years of relevant social work experience.
• Licensure:
  • Permanent Licensed Social Worker (LSW) preferred.
• Skills & Knowledge:
  • Strong clinical assessment, planning, and goal-setting skills.
  • Knowledge of state and county adult and child protective legal mandates.
  • Understanding of growth and development principles across the lifespan.
  • Strong counseling skills to help patients and families understand and accept medical recommendations.
  • Proficiency in Microsoft Excel and Word.
