- Interview patients and relatives to obtain relevant social history for medical and discharge planning.
- Assist patients with environmental and social challenges that may impede care and recovery.
- Serve as a liaison between healthcare teams, patients, families, and external agencies.
- Interpret and resolve social problems related to medical conditions and hospitalization.
- Collaborate with medical and nursing staff to develop and implement patient care plans.
- Education:
- Bachelor’s Degree in Social Work (BSW) required.
- Experience:
- 0-3 years of relevant social work experience.
- Licensure:
- Permanent Licensed Social Worker (LSW) preferred.
- Skills & Knowledge:
- Strong clinical assessment, planning, and goal-setting skills.
- Knowledge of state and county adult and child protective legal mandates.
- Understanding of growth and development principles across the lifespan.
- Strong counseling skills to help patients and families understand and accept medical recommendations.
- Proficiency in Microsoft Excel and Word.
