In this role, you'll play a vital part in helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors, helping us move forward with purpose and precision. Essential Job Duties:

  •  Communicate program purpose and health impact to clients, ensuring they understand available services and benefits.
  • Assist clients in developing and understanding care plans and goals, including navigating healthcare systems and securing needed services.
  • Conduct timely home visits and complete required documentation promptly.
  • Coach clients in managing chronic conditions and adopting healthy lifestyle behaviors through individual or group education.
  • Build community capacity by addressing social determinants of health and reducing cultural and socioeconomic barriers.
  • Collaborate with clinical and community partners to ensure coordinated, culturally competent care.
  • Participate in outreach and engagement activities to connect with new clients and promote services.

Requirements

Qualifications:

  • High School Diploma or its equivalent. 
  •  Experience working with culturally and economically diverse populations.
  • Successful completion of a Community Health Worker formal training program such as from a college or other education institution within one year of date of hire.   Maintain this certification. 
  • Successful completion of a Mental Health First Aid Training, preferred.
  • Ability to be insured on the commercial auto insurance policy based on driving record including active driver’s license and personal automobile insurance and reliable transportation. 
  • Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen 
  • Within the first 90 days of employment:  BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation.
