Description

Summary: Empower consumers to access and use needed services to meet self-determined goals. Plan referrals, linkage, monitoring and support. Plan discharges and provide advocacy assistance provided in partnership with consumers to support their biopsychosocial needs while in treatment.


Responsibilities:

  • Adhere to all Palmer goals, strategies, and values.
  • Maintain strict confidentiality in accordance with Palmer Polices and state and federal laws.
  • Role model appropriate behavior, professional boundaries, respectful communication, and personal appearance in accordance with Employee Dress Code.
  • Develop case management care plan with each consumer, including discharge planning.
  • Complete strengths-based assessments and generate needs list to be addressed.
  • Maintain case management documentation for adult and children, and provide complete and accurate records to Medical Records within time requirements.
  • Linkage, Referral, Follow-up, Monitoring and Advocacy between consumer and community resources.
  • Provide court advocacy, coordinating with consumer and clinical staff in regard to court hearings and DHS. Provide transportation and support. Follow agency policy and all federal and state laws during transports. Requires registration as agency driver with Human Resources.
  • Plan, implement and facilitate educational and skills groups.
  • Refer clients to outside sources, especially in preparation for discharge. Aid consumers in addressing their housing needs are met prior to discharge.
  • Serve as a member of the treatment team and report each consumer’s Case Management progress.
  • Assist with agency’s capability to manage co-occurring disorders, in compliance with the Integrated Services Initiative.
  • Enforce all facility rules fairly and consistently.
  • Maintain facility security and assist in basic crisis intervention.
  • Participate in ongoing in-service education programs and staff development programs.
  • Maintain knowledge of agency policies, procedures and training.
  • Maintain core competencies in relation to working with substance use disorders through continuing education and implement skills learned.
  • Perform other duties as assigned by supervisor and/or Director.

Requirements

Requirements:

  • Current Certified Behavioral Health Case Manager II certification from ODMHSAS
  • At least 2 years of experience in substance abuse treatment program, preferred
  • Knowledgeable in providing integrated evidence-based services for consumers with co-occurring disorders
  • Experience providing individual and group rehabilitation education.
  • Excellent verbal and written communications skills
  • Strong interpersonal skills
  • Computer skills include Microsoft Word, Excel, and Outlook
  • Current Oklahoma Driver’s License and acceptable driving record according to Motor Vehicles Records

Physical Requirements:

  • Ability to lift 30 pounds, sit for up to 8 hours/day, view computer screen for up to 8 hours/day.
  • Job requires twisting, lifting, standing, sitting, walking, stretching, kneeling, bending, and squatting.
