About the Role:

  • Permanent, full-time position providing crisis assessments, interventions, counselling, and psychoeducation, while coordinating access to psychosocial supports and community services tailored to individual needs in acute care services.

  • Proactively monitor and respond to changes in consumer risk status, ensuring timely, clinically appropriate escalation and intervention.

  • Foster strong collaboration with internal teams and external partners — including health professionals, crisis accommodation services, and the Homeless Health Assertive Outreach team — to ensure seamless, coordinated care.

  • Champion consumer advocacy by addressing personal needs, concerns, and preferences, in full compliance with the NSW Mental Health Act (2007; amended 2015).

  • Support a culture of learning and improvement by contributing to staff development, delivering training within the multidisciplinary team, and participating in ongoing service enhancement initiatives.
     

About You:

  • Degree in Social Work, eligible for membership of the Australian Association of Social Workers (AASW) and a full Australian driver’s licence.

  • Experience in community mental health, with confidence in acute and triage settings, risk assessment, and crisis intervention.

  • Committed to holistic, person-centred practice that includes families and carers as part of the care team.

  • A natural collaborator who builds strong, purposeful partnerships across services to deliver seamless, compassionate wraparound support for complex caseloads

  • You’re highly organised, with excellent communication skills and confidence using electronic medical records to manage complex caseloads.
