Hospital Sisters Health System (HSHS) is seeking a Community Health Outreach Specialist to join our team. The Community Health Outreach Specialist coordinates community health programming within respective assigned community. Collaborates with department leadership to develop, execute and evaluate initiatives based on Community Health Needs Assessment. Uses data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies, and environments. Participates in local and regional programming aligned with community health needs assessment priorities to develop and manage health education programs that help individuals, families and communities maintain healthy lifestyles. There is no premium pay for this position.

Position Specifics:
• Schedule: Full time, 40 hrs/wk, Monday-Friday 8:00am-4:30pm, some evenings and weekends
• Location: Hybrid, based in Green Bay, WI, travel to Sheboygan and Oconto Falls
• Compensation that aligns with your experience
Education Qualifications
  • Bachelor’s degree in public health, health education, epidemiology, or community health is required.
  • High School diploma, plus five years' experience in public health, health education, epidemiology, or community health may be considered in lieu of a Bachelor's degree.
Experience Qualifications
  • 3-5 years of experience in either health education, public health, or community-based leadership is required. (This experience is in addition to any experience in lieu of degree).

Pay Range:

$26.80 - $40.20

A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.
