Registered Nurse – Home Health

Branch: Teague, TX | Schedule: M-F 8-5, On call rotation weekends once a month | Coverage: Waco, TX and surrounding area

What You’ll Do as a Registered Nurse – Home Health

• Ensure all activities align with the vision of Elara Caring leadership and Home Health team standards

• Provide skilled nursing assessments and interventions based on patient/family needs, as documented in clinical records and interdisciplinary reports

• Manage a caseload by assessing, planning, coordinating, implementing, and evaluating individualized plans of care

• Conduct initial evaluations and ongoing re-evaluations to identify physical, psychosocial, and environmental needs

• Deliver follow-up visits as appropriate to patient, family, or facility requirements

• Communicate significant findings, changes in condition, and safety concerns to the Clinical Supervisor, physician, facility staff, and care team

• Revise care plans in response to patient needs and collaborate with the interdisciplinary team for continuity of care

• Protect patient and staff privacy, ensuring compliance with HIPAA standards

What You’ll Need

• Current, unrestricted RN license (state-specific)

• Minimum of 1 year of nursing experience in a clinical care setting

• Strong clinical assessment and critical thinking skills

• Ability and willingness to travel within branch/office coverage area (up to 50%)

• Reliable vehicle, valid driver’s license, and current auto insurance (per state requirements)

• Associate’s or Bachelor’s Degree in Nursing (preferred)

• Prior Home Health or Hospice experience (preferred)

• Experience with HomeCare HomeBase (preferred)

Reporting Structure

This position reports directly to the Clinical Manager or Clinical Supervisor.
