RN Clinical Team Manager – Home Health
Branch: San Antonio, TX | Salary | Schedule: M-F 8-5

Key Responsibilities
Provide leadership, guidance, and support to a clinical team, ensuring delivery of safe, high-quality home health care
Manage daily clinical operations, including staffing, scheduling, and patient assignments, with a focus on efficient and effective care delivery
Oversee compliance with all regulatory requirements, including Medicare/Medicaid home care policies and procedures
Implement and manage QAPI programs, ensuring continuous improvement in patient outcomes and team performance
Collaborate with physicians, patients, families, and interdisciplinary team members to coordinate and evaluate care plans
Monitor clinical documentation to ensure accuracy, timeliness, and compliance with regulatory and organizational standards
Mentor, coach, and develop staff to achieve professional growth and enhance team performance
Promote a culture of accountability, compassion, and excellence across the clinical team
Qualifications
Bachelor’s degree in nursing preferred
Current RN license in good standing
Minimum 3 years of nursing experience, with at least 1 year in the home health industry preferred
Strong knowledge of Medicare/Medicaid home care benefits, policies, and procedures
Valid driver’s license, current auto insurance, and reliable transportation required
Proven leadership, communication, and organizational skills

This position reports directly to the Director of Nursing or Clinical Administrator.
