RN Clinical Team Manager – Home Health

Branch: San Antonio, TX | Salary | Schedule: M-F 8-5

Key Responsibilities

• Provide leadership, guidance, and support to a clinical team, ensuring delivery of safe, high-quality home health care

• Manage daily clinical operations, including staffing, scheduling, and patient assignments, with a focus on efficient and effective care delivery

• Oversee compliance with all regulatory requirements, including Medicare/Medicaid home care policies and procedures

• Implement and manage QAPI programs, ensuring continuous improvement in patient outcomes and team performance

• Collaborate with physicians, patients, families, and interdisciplinary team members to coordinate and evaluate care plans

• Monitor clinical documentation to ensure accuracy, timeliness, and compliance with regulatory and organizational standards

• Mentor, coach, and develop staff to achieve professional growth and enhance team performance

• Promote a culture of accountability, compassion, and excellence across the clinical team

Qualifications

• Bachelor’s degree in nursing preferred

• Current RN license in good standing

• Minimum 3 years of nursing experience, with at least 1 year in the home health industry preferred

• Strong knowledge of Medicare/Medicaid home care benefits, policies, and procedures

• Valid driver’s license, current auto insurance, and reliable transportation required

• Proven leadership, communication, and organizational skills
