Full policy is provided upon request.<br /> <br /> <b>The impact you will make:</b></span></span></span> <ul> <li><span><span><span><span>Complete initial and on-going assessments and evaluation to identify the client’s level of functioning and ability to communicate, including OASIS assessment at appropriate time points.</span></span></span></span></li> <li><span><span><span><span>Combine knowledge of speech, hearing and language with planning and implementing programs for the management of communication disorders.</span></span></span></span></li> <li><span><span><span><span>Develop the plan of care in consultation with the physician and other care team members.</span></span></span></span></li> <li><span><span><span><span>Instruct other health team personnel, including Home Health Aides and the patient's family, in Speech Therapy Program.</span></span></span></span></li> <li><span><span><span><span>Participate in in-service education, utilization review and team conferences.</span></span></span></span></li> <li><span><span><span><span>Provide discharge planning, as appropriate.</span></span></span></span></li> <li><span><span><span><span>Evaluate effectiveness and outcomes of care / response to therapy interventions.</span></span></span></span></li> <li><span><span><span><span>Additional list of duties and responsibilities is provided in the Job Description</span></span></span></span></li> </ul> <br /> <span><span><span><b>Who you are:</b></span></span></span> <ul> <li><span><span><span><span>Master’s or doctoral degree in Speech-Language Pathology and meets qualifications commensurate with § 484.115(n) (Condition of participation:  Personnel qualifications / Speech-Language Pathologist).</span></span></span></span></li> <li><span><span><span><span>Current, active license as a Speech Pathologist by the California Board of Medical Quality Assurance.</span></span></span></span></li> <li><span><span><span><span>Core competency per skills checklist.</span></span></span></span></li> <li><span><span><span><span>At least one year of experience in speech therapy.</span></span></span></span></li> </ul> <br />
