We are seeking a highly skilled and forward-thinking Nurse Practitioner to join our team in a dual-capacity role as our Quality Improvement- Nurse Practitioner. This position combines direct patient care with strategic leadership in population health. The ideal candidate will be passionate about delivering high-quality, team-based care while leveraging data to identify trends, develop interventions, and support value-based care initiatives.

Position: Quality Improvement Nurse Practitioner (NP)

Wage Range: $120,000 - $160,000 annually, depending on experience

Role type: Hybrid - working both remotely from home as well as on-site (corporate headquarters or at a senior living community)

Schedule: 1.0 FTE, Monday – Friday 8:00am-5:00pm

Quality Improvement- Nurse Practitioner Functions and Responsibilities 

Clinical Care (50%)

  • Comprehensive assessments: physical exams, health history evals, cognitive assessments
  • Care planning: developing individualized care plans based on the members needs and preferences
  • Member education: provide education to members and families regarding disease management, preventative care, medications and health promotion. 
  • IDT: closely working with the primary care APP, MD, SW, CHN's, etc to ensure a coordinated approach to member care. 
  • Ensure the delivery of preventative services: immunizations, screenings, quality care standards. 
  • Member advocacy: advocating for the unique needs of the geriatric population. Member should always be in the forefront- ensuring care provided aligns with the best interest of the member. 

Population Health Leadership (50%)

  • Analyze clinical and operational data to identify population health trends, care gaps, and opportunities for improvement.
  • Co-lead our Quality Program and collaborate with data analysts, clinical leaders, and the Director of Initiatives Development and Delivery to develop actionable insights.
  • Design and implement population health interventions aligned with organizational goals and value-based care models.
  • Draft and refine clinical protocols and workflows that support team-based care delivery.
  • Serve as a clinical resource and change agent in the adoption of evidence-based practices and care models.
  • Monitor and report on key performance indicators (KPIs) related to quality, cost, and patient outcomes.

 

Qualifications for Quality Improvement Nurse Practitioner

  • Education: Master’s or Doctorate degree in Nursing from an accredited program. 
  • Licensure: Current and unrestricted Nurse Practitioner license in MN; national certification (e.g., ANCC, AANP). 
  • Experience: Minimum 3–5 years of clinical experience as an NP; experience in population health, quality improvement, or value-based care strongly preferred. 

Required Skills:

  • Strong analytical and critical thinking skills. 
  • Proficiency in using electronic health records (EHRs) and data visualization tools. 
  • Excellent communication and collaboration abilities. 
  • Demonstrated leadership in clinical innovation or care transformation

Preferred Qualifications

  • Experience with risk stratification, care gap closure, and chronic disease management programs. 
  • Familiarity with CMS quality measures, ACOs, or other value-based payment models. 
  • Certification in population health, healthcare quality, or informatics is a plus.
