A midlevel manager in Clinical Operations/Nursing Office, the Manager of Health Education and Risk Reduction Programs manages the planning, delivery, and integration of health education and risk reduction programs/services in the ambulatory care setting; manages the grants/external funding sources supporting such programs/services; and supervises assigned non-licensed care team members in direct service delivery.
This position manages a grant portfolio of 10-12 unique funding sources with combined annual budget of >$1M, the largest portion of which is funded through the federal Ryan White HIV/AIDS Program (RWHAP) and the State of Minnesota, e.g. Departments of Health (MDH) and Human Services (DHS).

- Program Management
- Develop and support a coordinated system of health education and risk reduction programs/services that collectively increase patient access and enhance the patient experience; engage necessary internal public health and clinical care functions to design and implement such programs/services.
- Function within multidisciplinary, matrixed leadership structure to effectively and efficiently integrate health education and risk reduction programs/services into ambulatory care model.
- Stand up and chair the HIV Care Continuum Roundtable (or its equivalent) to assure coordinated planning, delivery, and evaluation of programs/services along the HIV care continuum (prevention-diagnosis-referral to care-retention in care-viral suppression).
- Coordinate monitoring, evaluation, and continuous quality improvement activities for the program portfolio; actively incorporate lessons learned and best practices into health education and risk reduction programs/services; adjust and improve strategies to reach targets.
- Incorporate relevant trends, current research, and state/national frameworks into program/service design and CQI processes.
- Serve on relevant internal committees/taskforces, as assigned.
- Grants Management
- Work collaboratively within matrixed environment to proactively identify funding needs/priorities and relevant opportunities to support program priorities and goals; contribute to, and at times lead, the pursuit of strategic funding opportunities (under direction of Office of Advancement).
- Forecast sponsored funding transitions (growth, contraction); develop and activate plans to maintain necessary revenue streams to support program priorities and goals.
- Manage awards in portfolio throughout the grant lifecycle.
- Operationalize project portfolio’s annual work plans and budgets; assure targets are met on time and on budget; forecast, spend, and reallocate budgets, as necessary, to meet targets.
- Coordinate internal usage of data management systems specific to program portfolio (e.g. CAREWare, EvaluationWeb), including data entry, data quality, and data assurance.
- Track program compliance with all relevant contracts, rules, and regulations (including Ryan White HIV/AIDS Program, Minnesota Department of Health, and other granting agencies).
- Staff Management
- Provide strategic support, supervision, and thought leadership to assigned staff
- Attract, develop, coach, and retain high-performance team members; proactively identify and respond to staffing needs as programs grow/contract
- Build staff capacity where gaps exist; identify areas for staff professional development, and proactively create opportunities for such growth
- Facilitate team cohesion, collaboration, and processes; create a positive work environment that encourages communication, teamwork, and shared lessons learned; identify and address problems with prompt resolution
Knowledge, Skills and Abilities
- Ability to motivate others and create a team environment; experience supervising diverse teams. Ability to think critically and devise innovative solutions to meet program objectives and address gaps/challenges.
- Excellent writing, editing, and proofreading Impeccable attention to detail.
- Deep knowledge of and familiarity with gender/sexual health topics and trends; demonstrated ability to provide subject matter expertise/technical assistance to matrixed teams; knowledge of primary care service delivery in health center setting, including family planning/reproductive health and HIV specialty care.
- Ability to design and manage project plans and budgets; skilled in organizing, managing deadlines, and accomplishing objectives within parameters of approved plans and budgets. Understand and utilize principles and tools of continuous quality improvement, monitoring/evaluation frameworks, and logic models.

Supervisory Responsibility
Works independently with minimal direction and consultation from supervisor. Directly supervises assigned staff. May also supervise and/or direct contractors, volunteers, and/or students/trainees.
Work Environment and Physical Demands
The work environment varies based on assigned tasks. This role infrequently comes into contact with patients who may have contagious illnesses. This position is minimally active. The employee may periodically lift and/or move items under 20 pounds.
Position Type and Expected Hours of Work
This is a full-time position. Work hours largely conform to weekday (8-5) hours of operation, with negotiated flexibility to maximize productivity and fit with regular evening and weekend work.
Travel
Requires regular travel within seven-county metro area; requires periodic statewide and national travel.
Required Education and Experience
- Advanced degree in Public Health, Social Work, Nursing, Healthcare Management, or related field. BA/S and equivalent experience may be substituted for advanced degree.
- Licensed as a Registered Nurse in good standing within the state of Minnesota
- Minimum two years of experience in direct service delivery in a primary care and/or community health care delivery setting
- Previous direct staff supervision and program management experience
- 3-5 years of managing donor-funded public health services; direct experience managing Ryan White HIV/AIDS Program (RWHAP) and/or PEPFAR funding a plus
Preferred Education and Experience
- Proficiency in a language other than English; Spanish, Somali, Hmong preferred
- Relevant technical training/licensures, such as Licensed Social Worker, Registered Nurse, or other
- Knowledge of community health center ecosystem, and related best practices
- Experience managing sponsored/funded programs, including working with donor agencies, sponsored programs administration, and state/federal funding agencies
- Flexibility, including ability to function effectively in a changing environment
