Responsibilities:

- Proficiency with the MS office platform to include Word, Excel, Outlook, and PowerPoint, as well as other platforms

- Strong reading, writing and skills
- Must be comfortable in an ever-changing environment
Requirements:
- Update documentation for patients
- Provide necessary training to patients, home health aides, and nurses as required
- Communicates and build rapport to providers and facilities
- Conduct visits to patient’s homes to build and strengthen relationships with the patient and their family members.
- Filing compliance paperwork as needed from contractors
- Assist in the preparation and processing of patient assessments and care plans.
- Collaborate with care teams, patients, and their healthcare providers to obtain medical documentation for their home care renewals every six months
- ******Must undergo and pass a background check with the Department of Health******
