Compensation & Benefits
- Salary: $110,000 – $125,000 per year (based on experience + achievable monthly bonuses)

Key Responsibilities
- Conduct comprehensive patient assessments and create individualized care plans
- Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
- Collaborate with interdisciplinary facility teams to ensure coordinated care
- Educate patients, families, and staff on health management and treatment plans
- Complete documentation within 24 hours
- Ensure compliance with all applicable state and facility regulations
Qualifications
Qualifications
- Active Nurse Practitioner (NP) License in state of practice.
- 2 years of experience preferred but not required
- Preferred experience in:
- Skilled Nursing Facilities (SNFs)
- Hospice and/or palliative care
- Emergency department, ICU/SICU, or hospitalist settings
- Proficiency in Electronic Health Records (EHR)
- Strong communication skills and a collaborative, compassionate mindset
Schedule & Location
- Schedule: Monday–Friday, 8 AM – 5 PM
