Compensation & Benefits

• Salary: $110,000 – $125,000 per year (based on experience + achievable monthly bonuses)

Key Responsibilities

• Conduct comprehensive patient assessments and create individualized care plans
• Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
• Collaborate with interdisciplinary facility teams to ensure coordinated care
• Educate patients, families, and staff on health management and treatment plans
• Complete documentation within 24 hours
• Ensure compliance with all applicable state and facility regulations

Qualifications

1. Active Nurse Practitioner (NP) License in state of practice.
2. 2 years of experience preferred but not required
3. Preferred experience in:
   • Skilled Nursing Facilities (SNFs)
   • Hospice and/or palliative care
   • Emergency department, ICU/SICU, or hospitalist settings
4. Proficiency in Electronic Health Records (EHR)
5. Strong communication skills and a collaborative, compassionate mindset

1. Schedule: Monday–Friday, 8 AM – 5 PM
