ABOUT THE ROLE

We’re searching for a dynamic, hands-on Clinical Care Coordinator to lead health and wellness operations in our Assisted Living and Memory Care units.

You’ll be more than a nurse — you’ll be a mentor, problem-solver, and advocate, ensuring every resident receives personalized, compassionate care while empowering your team to deliver their very best.

 

WHAT YOU’LL DO

•  Ensure top-quality care on every shift (and step in to assist as needed).
•  Lead, coach, and inspire nursing staff, medication techs, CMAs, and caregivers.
•  Train team members with engaging, real-world learning.
•  Maintain accurate charts and documentation.
•  Assess, plan, and update individualized Personal Service Plans.
•  Spot and address health concerns early to help residents stay in their home longer.
•  Oversee medication management, training, and audits.
•  Share on-call and Manager-on-Duty responsibilities.
•  Assist with onboarding new team members.

 

WHAT YOU BRING

✔ RN or LPN/LVN license (current & in good standing)
✔ 3+ years’ relevant healthcare experience (supervisory a plus!)
✔ Strong communication & leadership skills
✔ Computer proficiency
✔ Confidentiality & professionalism
✔ Ability to pass background & drug screening
