Responsibilities include interviewing patients and relatives to gather social histories relevant to medical planning, assisting with environmental difficulties, and acting as a liaison between medical staff, patients, families, and outside agencies. The goal is to interpret and resolve social issues that impact medical care and hospitalization.Key Responsibilities:

• Interview patients and relatives to obtain social history pertinent to medical problems and planning.
• Assist patients with environmental and social difficulties that interfere with optimal healthcare benefits.
• Serve as a liaison between medical and nursing staff, patients, families, and outside agencies.
• Interpret and assist in resolving social problems related to medical conditions and hospitalization.
• Support patients in understanding, accepting, and following medical recommendations through counseling.

Qualifications & Requirements:

• Experience: 0-3 years of relevant healthcare or social work experience required.
• Education: Bachelor's Degree in Social Work (BSW) required.
• Licensure: Permanent LSW licensure preferred.
• Skills:
  • Strong clinical assessment, planning, and goal development skills.
  • Knowledge of state and county adult and child protective legal mandates.
  • Understanding of growth and development principles across the lifespan.
  • Excellent counseling skills to support patient understanding and adherence.
  • Proficiency in Microsoft Excel and Word.
