We are seeking a compassionate and dedicated Social Worker to assist patients and their families by addressing social challenges, coordinating resources, and facilitating the best possible outcomes during hospitalization.Apply now — make a meaningful difference in patient care and support!Position Overview:The Social Worker provides essential support for patients with complex psychosocial needs, helping them navigate social support programs and community resources. Responsibilities include interviewing patients and relatives to gather social histories relevant to medical planning, assisting with environmental difficulties, and acting as a liaison between medical staff, patients, families, and outside agencies. The goal is to interpret and resolve social issues that impact medical care and hospitalization.Key Responsibilities:
  • Interview patients and relatives to obtain social history pertinent to medical problems and planning.
  • Assist patients with environmental and social difficulties that interfere with optimal healthcare benefits.
  • Serve as a liaison between medical and nursing staff, patients, families, and outside agencies.
  • Interpret and assist in resolving social problems related to medical conditions and hospitalization.
  • Support patients in understanding, accepting, and following medical recommendations through counseling.
Qualifications & Requirements:
  • Experience: 0-3 years of relevant healthcare or social work experience required.
  • Education: Bachelor's Degree in Social Work (BSW) required.
  • Licensure: Permanent LSW licensure preferred.
  • Skills:
    • Strong clinical assessment, planning, and goal development skills.
    • Knowledge of state and county adult and child protective legal mandates.
    • Understanding of growth and development principles across the lifespan.
    • Excellent counseling skills to support patient understanding and adherence.
    • Proficiency in Microsoft Excel and Word.
