Title: Mental Health Clinician
Reports to: Director of Clinical Operations
Job Objectives:
We are seeking a dedicated and compassionate Mental Health Clinician to join our team delivering community-based services through New York State’s CORE, which stands for Community Oriented Recovery and Empowerment, program. This role focuses on providing Community Psychiatric Support and Treatment, or CPST, a Medicaid-funded service designed to help individuals with behavioral health needs gain skills, improve functioning, and achieve greater independence within their communities.
As a CPST provider, you will work with clients in a variety of settings, including their homes, community spaces, and our agency site, to offer flexible, person-centered care. This position is ideal for a Licensed Master Social Worker (LMSW) who is passionate about promoting recovery, reducing hospitalizations, and helping clients navigate life’s challenges with dignity and empowerment.
You will play a vital role in supporting clients as they develop coping skills, build resilience, and connect with community resources that foster independence and well-being. Your work will focus on creating a safe, respectful, and collaborative environment where clients feel heard, valued, and motivated to achieve their personal goals. Through compassionate advocacy and evidence-based interventions, you will help individuals overcome barriers, improve their quality of life, and move toward sustained mental health recovery.
Direct supervision of stated positions and oversight of stated programs are periodically re-evaluated and may change subject to the discretion of Executive Director.
Responsibilities:
Client Services – CORE CPST Program :
· Carry and manage a caseload of clients enrolled in CORE CPST services.
· Deliver person-centered, strength-based mental health interventions in the community or home settings.
· Conduct clinical assessments to identify client needs, goals, and support systems.
· Create, implement, and revise individualized treatment plans.
· Collaborate with clients, families, and interdisciplinary professionals for holistic care.
· Document services and maintain compliance with Medicaid and agency requirements.
Intake & Assessment:
· Perform intake interviews and diagnostic evaluations for new referrals.
· Conduct assessments for agency programs and supported housing services.
· Provide clinical recommendations and assign appropriate levels of care.
· Complete documentation and LPHA assessments per regulatory guidelines.
General Support:
· Participate in team meetings and agency committees.
· Support other duties as assigned by leadership.

Requirements
Qualifications
· Applicant must possess one of the following
o License in Social Work (LMSW, LCSW)
o License in Marriage and Family Therapy (LMFT)
o Licensed Mental Health Counselor (LMHC)
o Licensed Creative Arts Therapist (LCAT)
o Licensed Psychologist
o Mental health counseling: 1 year.
· Experience using Motivational Interviewing and trauma informed practices.
· Experience navigating an Electronic Health Record.
· Experience working in community mental health, residential settings, or with families and children.
· Strong skills in assessment, treatment planning, and clinical documentation.
· Excellent interpersonal, communication, and collaboration skills.
· Familiarity with trauma-informed care and evidence-based practices.
· Bilingual Spanish-speaking candidates strongly preferred.
I. Terms of employment/benefits:
· Exempt.
· Forty (40) hours per week.
· Paid time off, holidays, and sick leave according to MHA policy.
· The position requires a valid driver's license to meet clients in the community or at their residence. In the case that a program vehicle is not available, mileage reimbursement is available at the federal rate.
