RN Care Coordinator

Description

Serving communities in Vermont (Windsor, Windham, Bennington counties) and New Hampshire (Sullivan and Cheshire counties), North Star Health is the primary care resource for a population of over 65,000 residents. NSH is a Federally Qualified Health Center (FQHC) providing service at and through a variety of service locations in South Eastern Vermont and South Western New Hampshire.

The Care Coordinator Will:

• Conduct timely and accurate assessment of the patient/ client clinical, social, functional and continuing care needs through direct observation, data analysis and interviews in order to evaluate the need for intervention. 
• Establish immediate, short term, and long-term care needs with the patient. 
• Engage in quality through value-based care delivery. 
• Engage in and promote communication at all levels. 
• Demonstrate competency and understanding of the specific knowledge and skills required for effective care coordination.

Requirements

• Experience with Care Planning
• Two (2) year's nursing experience
• Excellent communication skills
• Proficiency with Microsoft Office, computer skills
• Current LPN License
• One (1) – Two (2) years’ experience in case management (preferred)
• Current RN License (preferred)
