As a Service Coordinator, you will be an integral part of a collaborative team focused on delivering seamless services and programs to our clients and customers. If you are looking for a company that is constantly moving forward in pursuit of successful outcomes, get ready to love it here!

What you’ll be doing: 

  • Review referral requests to ensure that it meets established criteria
  • Conduct intake interviews with clients to obtain their consent and talk with them about the program
  • Ensure data is collected and input as required
  • Attend meetings with referral source as required
  • Document and escalate service issues
  • Monitor the work of assigned clinicians to ensure adherence to specific standards and timelines and provide support to our clinician network to ensure they are managing expectations
  • Add to a positive work culture by identifying efficiencies and solutions to any issues
  • Liaise with the client/patient, third parties (including insurers), the employer and various health care professionals
  • Act as the first point of contact for service providers and corporate customers and help manage and track complex cases 
  • Maintain database(s) and client/case files with a high level of accuracy and confidentiality 

What we’re looking for:

  • Bachelor’s degree in a related field or an equivalent experience and training
  • Case Coordination and or Case Management experience is a major asset
  • Expertise in Disability Treatment and Management principles, practices and processes
  • Proficiency with Microsoft Word, Excel and PowerPoint
  • Excellent verbal and written English communication skills
  • Effective self-directed work style with the ability to work with minimal supervision
  • Strong administrative and customer service skills
  • Effective interpersonal skills; ability to communicate with a variety of stakeholders
  • Ability to efficiently manage multiple demands under time constraints and deadlines 
  • Adaptable to changing priorities and contribute effectively to a positive work environment
