JOB SUMMARY
As a patient-center and quality improvement focused organization, TJRH continuously enhances the health and well-being of people in our The population health wellness nurse plays a critical role in the delivery of quality medical care. This position requires a person with strong clinical skills along with assessment and communication skills. This position will provide clinical care in collaboration with a multidisciplinary team to achieve positive patient outcomes.
JOB REQUIREMENTS
Minimum Education
§ RN required, bachelor’s degree preferred
§ Holds an active Registered Nurse license, and is eligible to practice in the state of Kentucky.
Minimum Work Experience
§ 3-5 years of clinical experience

ESSENTIAL FUNCTIONS
§ Participates in a multidisciplinary team to develop and implement care management strategies that eliminate care gaps, and fragmentation of healthcare services provided.
§ Establishes and maintains positive relationships with all internal and external customers.
§ Excellent communication skills.
§ Demonstrates leadership skills and the ability to direct the design, implementation and ongoing modifications of the population health program.
§ Demonstrates strong organizational skills.
§ Integrate evidence based clinical guidelines for preventative care.
§ Excellent interpersonal skills, patient engagement skills and the ability to work independently and collaboratively are key requirements to this role.
§ Maintain a patient registry for identified wellness visits.
§ Conduct wellness visits with physician oversight, which includes: review and update medications, problem list, allergy list, risk factors etc.
§ Coordination and collaboration with primary care team.
§ Monitor adherence to treatment plans, self-monitoring, scheduling necessary appointments.
§ Assess and provide support for patient self-management.
§ Maintain all required documentation of activities.
§ Provide consistent communication to the care team to evaluate patient/family status.
§ Closely supports patient through transitions of care.
§ Attend medical and specialty appointments with patient when appropriate.
§ Attend appropriate meetings, rounding, and conferences as needed.
§ Participate in quality improvement activities as assigned.
§ Other duties as assigned.
