This Per Diem opening is perfect for Certified Nursing Assistants (CNA) or Licensed Nursing Assistants (LNA) who want to pick up extra shifts or just want to work two days a week. The role offers a unique opportunity to gain valuable experience in both inpatient and outpatient Hospice care, working alongside a compassionate and supportive team.

Hours/Shift: Per Diem/Varies - minimum of two shifts per week commitment
Qualifications:
- Must be at least 21 years of age or older
- High School Diploma or Equivalent
- DPS Level One Fingerprint Clearance Card, required within 20 days of hire
- BLS Certification through American Heart Association or American Red Cross
- Arizona Food Handlers Card
- Arizona Driver's License and clean driving record
- For those candidates with a Certified Nursing Assistant (CNA):
- Current valid Arizona CNA License from the AZ Board of Nursing
- It is strongly preferred that CNA applicants plan to obtain an LNA
- For those candidates with a Licensed Nursing Assistant (LNA):
- Arizona Licensed Nursing Assistant (LNA) from the AZ Board of Nursing

Key Responsibilities:
- Responsible for attending to the daily living and safety needs of an assigned group of patients providing age appropriate care.
- Assist registered nursing personnel in the care of patients by performing various patient care activities and related non-professional services, and performs hourly rounds.
- Performs rounds, designated admitting tasks, assures patient belongings are accounted for on admission and transfer and returned at discharge.
- Collaborates and coordinates with other departments to provide timely effective care consistent with individual’s needs, choices and preferences.
- Obtains and records vital signs, heights and weights and meets patient’s personal hygiene, basic needs promptly, ensures that the patient is comfortable and necessary supplies are available and in reach.
- Keeps patient environment safe and tidy and follows infection control procedures.
- Demonstrates effective communication skills with children, adolescents, adults and geriatric patients to include use of appropriate therapies such as play, written, and picture board.
- Reports changes in patient’s condition, out of normal values, patient/family concerns or complaints to nurse, charge nurse and/or supervisor.
- Applies AIDET as well as other evidence-based behaviors.
- Participates in developmental and organizational initiatives for process improvement and quality assurance as requested.
- Completes bedside shift report and performs and accurately documents procedures as directed by the RN / LPN, accurately and timely.
- Maintains patient and staff confidentiality and expected productivity to average no less than 5-6 patients per day.
- Assists in the orientation of new staff as requested.
Work Requirements: Able to handle multiple priorities simultaneously, possess good oral and written communication skills, care for patients with infectious diseases, utilize standard office equipment; Able to assist with the movement of an adult patient to 300 lbs, able to stand, bend, stoop, kneel, and reach, while working with patients for the entire shift; sit, stand, and walk 3-5 hrs daily; Expected duties have routine potential for exposure to blood, body fluids or tissues.
