Position Summary:

The Community Concurrent Disorders Program (CCDP) is a wraparound service for individuals experiencing mental health and substance use issues. Trillium Health Partner’s short-term case management program utilizes a person centered approach that is grounded in harm reduction and recovery principles. 

The successful candidate will work collaboratively with the client to promote positive choices through empowering people to become an active part of their own recovery journey. 

Key Qualifications:       

  • Professional qualification in a health discipline with addictions specific training required.
  • Certification through the Canadian Addiction Counsellors Certification Federation considered an asset.
  • Minimum 2 years experience working with adult clients with mental illness and substance use issues.
  • Demonstrated ability to assess clients, implement a service plan and facilitate referral to treatment programs within a short –term model of care.
  • Experience with utilizing motivational interviewing skills and techniques. 
  • Knowledge of community resources. 
  • Excellent communication, organization, presentation and problem solving skills.
  • Valid driver’s license, relevant insurance and use of own vehicle.
  • Demonstrated ability to work independently and cooperatively with community partners.
  • Ability to speak a second language is considered an asset.
  • Proficient computer skills.
  • Reliable, flexible and collaborative team player.
  • Positive work and attendance record essential.
