WORK LOCATION(S): Middletown, NY

STATUS: Full Time

SCHEDULE:  Monday: 10:30 AM – 7:00 PM | Tuesday to Friday: 8:30 AM – 5:00 PM

General Purpose:

The Lead LPN of Urgent Care will provide an oversight of the nursing and medical assistant patient care provided in the department in collaboration with the Director of Nursing and the Chief of Urgent Care. 


Description of Duties:

  •  Provide supervision of Urgent Care Nurses and Medical Assistants.
  • Provides direction and/or oversight of triage process for patients walking in or calling in to the Urgent Care department within NYS LPN scope of practice and seeks directive from UC provider or RN when appropriate.
  • Performs huddles with providers and nursing staff throughout shift to assist with managing patient flow based on triage, volume of patients and the needs of each provider.
  • Ensure the department’s proxy in ECW is assigned and addressed in a timely manner; and is free of outstanding phone notes/messages, orders, patient results, refills.
  •  In collaboration with the Director of Nursing, create, implement and supervise the new nurse and medical assistant orientation process.
  •  Assists the Director of Nursing in yearly staff education, training, competency, and evaluations.
  • Maintain appropriate daily staffing needs for the department. 
  • Complete nursing care team monthly schedules and provide coverage when needed. 
  •  Ensure that nursing and support staff remain current with screening duties and policies as per Cornerstone policies and procedures.
  • Strictly adhere to safe nursing practice including but not limited to Infection Control, National Patient Safety Goals, Medication Management and Waived Testing set forth by Cornerstone policy and in compliance with Joint Commission and OSHA.
  •  Provide specific nursing skills as ordered by medical provider and in accordance with NYS LPN scope of practice. Includes, but is not limited to, safe medication administration, vaccinations, EKG, Prescribed treatments, patient monitoring, and all point of care testing.
  • Perform random chart reviews and routinely monitor department flow for efficiency and work with Director of Nursing to make changes when needed.
  •  Monitor staff adherence to the patient’s experience/satisfaction and provide feedback or additional training when necessary.
  • In concert with department chief, participates in CQI projects and monitors progress on quarterly basis. Develop nurse/MA interventions directed at CQI project and develop staff education as necessary.
  •  Ensure daily, weekly, and monthly safety and clinical inspections are completed and submitted to the appropriate party. Conduct random unannounced unit inspections to ensure adherence.
  •  Ensure that all pharmaceuticals, both stock and samples, are stored according to policy, and inventoried monthly to check expiration dates and recalls. 
  •  Responsible for coordinating care and providing patient education based upon their needs. 
  •  Attend and participate in monthly department, All Staff and other required meetings. Facilitate department staff huddles as needed.
  •  Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Policy and Procedure Manual and Employee Handbook.
  •   Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations. 
  • Perform other duties as assigned.

Requirements

  • Successful completion of accredited LPN program 
  • BLS Certification
  • NYS Education Department Licensed Practical Nurse 
  • Bilingual
